Lack of communication due to cognitive impairment and lack of health literacy clearly affects the medication adherence of the older patients, especially ethnic minority in origin. More than sixty percent of the minority population does not comply with the prescribed drug regimen in the USA. Non-verbal communication seems to be effective in such settings. Effects of cognitive impairment on the reliability of geriatric assessments has been studied recently (Phillips et. al, 1993) to explore the relationship between cognitive status and reliability of multidimensional assessment data. Monolingual and Bilingual communication between patients with dementia and their care givers have been studied and have shown that the relationship between a demented terminally ill patient and the care giver is an important factor which forms the foundation of the interaction between them. This communications is complex with different aspects of perception and practice (Sirkka, 1996). Non-speaking patients are the worst affected and it is important to improve their communication to allow them receive care and comfort. The ethnic old patients in Canada have been shown to have remained an isolated group as aliens in their own land. This ethnic population has been shown to be unable to communicate with their care givers (Saldovt, al 1994). Problems developed in such patients without interpreter services have been termed serious. Thus, the problems encountered in communication between health care professionals and patients can be based on cultural needs and cognitive status; Cognitively impaired or comatose are often neglected. Studies on demented terminally ill patients to evaluate the time care givers provide for direct patient care have showed that more time is being spent with non demented than with the demented. Another studies on communication with severely demented patients by the health care professionals have shown that care of such patients need clear communication during care procedures. Thus, use of non-verbal communication skills (NVC) to improve care, especially with people who have learning disability has been assessed in a study (Chambers, 2003). The study outlines a health care professionals’ diagnosis of altered non-verbal communication and a new wellness diagnoses for enhanced non-verbal communication with detailed discussion on use of NVC with people with comprehension difficulties. The study stresses on the fact that health care professionals can be important in enhancing the non-verbal skills of the patient to help them communicate. Adding support to the view, the importance of improving communication by touch has been documented (Vortherms, 1991). The article views touch as an integral aspect of care, with the language of touch including tactile symbols of duration, location, action, intensity, frequency and sensation. The article classifies touch as affectional, functional and protective. The article stress that age is not a category to decide upon touch in terms of reduced needs of touch. These drugs also pose the risk of seizures. With medical litigations on the rise, the interventions should be based on the competence of the patient. However in psychiatric cases there seems to be a group of individuals who are marginally competent. This group seems to lie in-between the two extremes of competence and incompetence and competence in this case thus appears to be a matter of degree. Mentally retarded persons who have some understanding of the reality and are able to express their wishes and desires can also be considered marginally competent. Mentally ill individuals whose illness has not completely impaired their understanding and capacity to express their wishes and desires are also considered marginally competent. These individuals are not incompetent though they suffer from specific deficits due to destroyed faculties. These marginally competent individuals make a significant group and recognizing the existence of such group of marginally competent individuals will help define competence better towards the documentation of informed consent and advance directives during interventions. Cognitive impairments and low health literacy pose a serious barrier on the reliability of patient assessments. Hence, there is a need for training the health care professionals on communication skills with reference to cognitive impairments and designing awareness programs for such patients towards better health care. GOALS AND OBJECTIVES The main Objective of the study is to evaluate self-reported medication compliance in patients 50+ years of age and to compare physician assessments of patient’s adherence to a prescribed drug regimen provided on the â€Å"Predictive Score Sheet† to data obtained in telephone survey. The main goal of the study is to administer Medication Adherence Telephone Survey to all patients that completed the initial RAND SF-36 Survey, to complete double entry of data using SPSS software, to design and implement a statistical analysis plan to address the stated research question and to understand the effects of self-reported data on the validity of a research study MATERIALS AND METHOD In such a context, a demographic survey and two health assessments were administered to a convenient sample for a consecutive two week period utilizing CLOX, an executive clock drawing test to screen for cognitive impairment and the Rapid Estimate of Adult Literacy in Medicine (REALM) test to measure health literacy. A stimulated patient chart reviews and a telephone survey to record medication adherence behavior was conducted. One hundred cognitively impaired patients with treatment-resistant schizophrenia referred for Clozapine therapy were studied over the period of two weeks of treatment. Clozapine is a tricyclic dibenzodiazepine derivative 8-chloro-11- (4-methyl-1-piperazinyl)-5H-dibenzo [b, e] [1,4] diazepine. It is available as 25mg and 100 mg tablets for oral administration. Clozapine is widely used in refractory Schizophrenia. There seems to be a cultural and geographical variations in dose response relationship and tolerability to the drug making the dose response unpredictable. The effective and tolerable range is between 100 mg to 900 mg, which makes it difficult to make a predictive, significant and valid correlation. There has been no positive correlation with serum level too. Adherence to medication is important in clozapine therapy for results. The study was open level, naturalistic, and prospective, involving all patients, consecutive one hundred, referred for Clozapine during the year 2006. Data was obtained from patients and key relatives after informed consent. After therapy began, patients received a follow-up evaluation on day 4, 8 and again on the last day of treatment. The patients included 62 men and 38 women of low to middle socio-economical class, ranging in age from 52 to 56 years, averaging 54 years of age. 38% were married, 43% were single, and 19% were separated. The duration of illness prior to the study ranged from 5 to 21 years, averaging 7 years. Patients’ duration of illness before first psychiatric contact ranged between 5 months and 46 months, averaging 20 months. Prior to the study, 55% of the patients had a history of hospitalization, averaging 2 prior hospitalizations and the mean duration of hospitalization was 2 months. A history of violence was recorded in 46% of patients and 72% had a history of behavioral disruption. 28% of the patients had attempted suicide. Some were unable to function in society- 66% were socially disabled and 74% had little or no occupational functionality. One fourth of the patients had a positive family history of some psychological disorder. Schizophrenia was diagnosed in 16% of those relatives: 5% in siblings, 8% in parents, and 3% among offspring. Other psychological disorders, including alcoholism, depression, and suicide attempts were confirmed in first-degree relatives among 9% of the patients.

The region, called India included areas administered by the United Kingdom. The princely states were ruled by individual rulers under the paramountcy of the British Crown. The political union was officially called the Indian company after 1876. It issued passports under that name. The result of this change was that it was a founding member of the League of Nation and the United Nation. Furthermore it was a member nation of the Summer Olympics in the years 1900, 1920, 1928, 1932, and 1936. When the rule of the British East India Company was transferred in the person of Queen Victoria, the system of governance was instituted. When the British Indian Empire was portioned into two sovereign dominion states, the Union of India and the Dominion of Pakistan, it lasted until the year 1947.   In the year 1937 the province of Burma became a separate colony. This province was located in the eastern region of the Indian Empire. Moreover it gained independence in the year 1948. The Geographical extent of the Raj India, Pakistan, and Bangladesh over all these regions the British Raj extended. At various times it included Aden Colony, Lower Burma, Upper Burma, British Somaliland, and Singapore. Only one region was directly administered by the British Crown from 1937 until independence in the year 1948. This region was Burma. In 1802, Ceylon was ceded to the United Kingdom under the Treaty of Amiens. Ceylon was never part of British India, but it was a British Crown Colony .The British having fought wars with the kingdoms of Nepal and Bhutan. They signed treaties with them. The meaning of The Raj The Raj is the name of the British rule in India. This expression was derived from the Sanskrit term raja. The meaning of raja is king. Many British merchants made a fortune in India and they return home, because they were derided by those in British high society as nabobs. Nabobs was the title for an official under the Moguls. The British public was fascinated by the tales of life in India. They liked exotic Indian scenes, for instance a drawing of an elephant fight.   This drawing appeared in books published in London in the 1820s.   Raj means rule. This word is of Indian languages. British Raj means rule by the British in India. The Republic of India, Pakistan, Bangladesh and Burma were all part of the British Indian Empire. At that time, it was known as the Indian Empire and sometimes now spoken of as the British Raj, which is a term of history. This rule was before 1947. The British Raj is used to talk of the direct British rule over areas which had been conquered by the British. This is known as British India. Undivided India is another term. It is used to mean the whole area of British rule. It does not take in Burma. From 1937 Burma was a British colony on its own. The colony of Aden came under the same government in India. It came from 1858 to 1937.Furthermore the British Somaliland came under the same government from 1884 to 1898 and Singapore from 1858 to 1867. Today Somaliland is called Somalia. The British rule ended in Pakistan and the East Bengal region on 14 August 194. Later the British rule ended in the rest on 15 August 1947. Jammu and Kashmir had not been under direct British rule. Over this area India and Pakistan have gone to war, and it is not divided between them. The beginning of the Raj In the year 1858, the British Crown was established in India. In this paper, I will try to suggest some solutions to improve Kmart as a company as a whole and help increase its sales. First of all, Kmart needs to improve its marketing technigues. The problem that it’s facing right now is decreasing sales. What I think it should do is lower the prices. It will definitely attract more customers. For example, the reason that one of Kmart’s biggest competitors, Wal-Mart is doing so well is because Wal-Mart is known for its very low prices. Also, I think that Kmart should increase eye-capturing advertising. Not only should they increase the advertising, but they should try to appeal to different kind of customers. Ernesto â€Å"Che† Guevara’s work is entitled Reminiscences of the Cuban Revolutionary War. He elaborates on how Cuba was revolutionized and what happened during those days. It was published in Australia in 2006 and runs to 314 pages. Guevara is conscious of his place in history and writes for posterity, to preserve a history of the armed struggle against the tyranny of the dictator Batista. The revolutionary icon Ernesto Guevara states that the Cuban Revolution has had a very important role in bringing consciousness and liberty to the Latin America region.   Because of the Cuban revolution the country has become one of the most influential nations in the world. Cuba has achieved its goal of the gradual eradication of illiteracy and unemployment is solved. Social security is also guaranteed and for the young there is the assurance of education. Aside from all the hindrances and challenges that Cuba had experienced before, the country still manages to stand up and fight for their rights and liberties through social revolution.  Socialism is said to be improved and will be accredited for the duration of the Cuban Revolution. Even through the darkest days and the blockades that were done to Cuba, the people did not stop their revolution. Guerilla warfare played an important role in the Cuban Revolution according to Guevara’s writings. Guevara’s guerrillas were able to overcome superior firepower and succeed over superior forces by living off the land and utilizing hit and run tactics. They knew where they would attack next and the government forces did not. They were trusted by the people and therefore were better organized politically by listening to the people. Guerilla warfare was a rebellious movement which was formed to overcome economic, political and cultural adversities. It promotes changes which aim to eradicate corruption in the government, to stop tyranny, repression and remove the power of aristocratic elites. Guevera relates that in liberated territories the need for health care, industry and education is basic and paramount. Before the Cuban revolution there was exploitation and cultural repression which made clear the division and inequality of wealth within the nation. In this environment the rich became richer and the poor became poorer. From my own perspective it seems that people of Cuba became radical about what was happening in their society, and that made rebellion or insurgency a viable option. According to Guevara’s writings, violence should only be an option if there is an illegitimacy of the government and the people of that nation believe that there is really injustice and inequality in their society.   Local radical tribunals were formed to lead what they call people's justice, to try   to solve public arguments in Cuba. Cuba became the center of international liberalism because of this, but the truth is it was really different. Ernesto Guevara was an influence in building a new political and social government in Cuba. He constantly utilized the teachings of Marxist Leninism leadership in his revolution, but he never confirmed this no did he deny it. Che was very willing to negotiate with the communists in Sierra, and he was likewise intolerant of Fidel Castro’s aggression and distrust of them. But still, Guevara’s attitude toward Castro made him realize that revolution must be done in Cuba and he must apply Marxism. An analysis of Marxism shows that classes are the central players in the process of history, yet for Guevara these classes are lacking and are replaced by the revolutionaries. Guevara used Marxist themes in his writings, and he used the philosophy of Karl Marx in his own theories of economics. In this vein it is also necessary to say that Guevara embraced not only communism but a particular branch of it, called Stalinism. He applauded the Soviet invasion of Hungary that crushed the workers’ uprising there in 1956 and rejected   Khruschev’s denunciation of Stalin’s crimes. Politically he seemed to be a Guevarist. I would say that Guevara played the biggest role in the Cuban revolution in political consolidation and the formulation of economic strategies. Unlike what Castro had believed, being a pragmatist, Guevara used his power in the leadership towards the creation of a powerful and independent state. Guevara did not favor nor did he totally support Fidel Castro’s pragmatic leadership, and he immersed himself in his writing, deciding the best method to refute Castro.   He read a work entitled Eastern European Handbook On Planning during this period. Guevara found that the key to Cuba’s survival is industrialization and economic growth. In his speeches on the economy between 1959 and 1965, his persistent themes were the importance of planning and development. According to Che, there is a need for industrialization and diversification and a need for consequently winning workers over to the idea that they must delay their anticipation of immediate material enhancement in their lives. In addition, Guevara’s analysis of the economy was influenced by Soviet thinking. The state is given authority over the revolutionary army; the guiding tenet of economics is the market, and the perception is that of a beleaguered   state defending its survival in opposition to the looting marketers. But according to Guevara, he knew the dangers of isolation. He then pursued a version of internationalism that takes into consider the problems unique to Cuba when it comes to its dependency. Therefore, internationalism is just a part of the problems that Cuba experiences during the revolution. Knowing that Cuba exists in a global milieu and by the stability of internal forces, then what political, social and economical alterations can make it stable, Guevara asks. For Ernesto Guevara, the response is, of course the Cuban Revolution.  He does not consider the production and exportation of revolution elsewhere. According to him, the only thing that Cuba can export is their political and economical state. Guevara is a most compelling and convincing revolutionary leader. Aside from his health problems, he disregards the material, and his undying dedication evokes the near worshipful devotion of others. At this period, he becomes at ease with Fidel Castro.. Being recommended as the Director of the National Bank made him the center of economic policies. Then Guevara formed the National Institute of Agrarian Reform which he designed for economic transformation. It can be said that agrarian reform becomes the basis of change and the peasants become the first liberating army. Starting with a more competent agriculture, and the eradication of joblessness, Cuba is now ready to become an industrialized country. Guevara also predicts that in five years, his country will be adequate in the production of textiles. It will be able to develop mines for nickel, iron and manganese production. Further, Cuba will have its first car plant up and running. Because of Cuba's economic organization, its reliance on sugar exportation, its need for spare parts, machinery and industrial plants, and also due to the drain of professionals and technicians, it is possible that Che's projections are very idealistic. Politically speaking, Cuba will not work or function independently for long, and formation of socialism in a hostile capitalist hemisphere was unattainable and utopian thinking. Che made his predictions and published them in an article during March of 1962   as he introduced the socialist theory. Simplistically, the impossibility of finding spare parts, the lack of technologies and the shortness of immediate resources needed to raise productivity could only be realized by an intensified management of labor.  Guevara realizes the absurdity of animosity among the leadership and comes to understand what can be lost in the face of fraternal rivalry, and what is to be gained by a cold realistic approach Guevara’s writings are expository and sensitive as to how the revolution in Cuba came about. He also explicitly reveals the scenario of the various occurances during in those days.   Cuba is obviously a nation built on revolution that has made a major impact on the world. The Cuban Revolution might well be considered the greatest contribution to the world of liberalism and revolution. Many transformations and drastic changes have occurred in Cuba. Ernesto Guevara is certainly an icon of revolution. He sets a standard for true revolution through his life, his analyzations and his writings. His writings on the intricate workings of the Cuban revolution is a great contribution to Latin America history for it is stated in his writings what he believes and how he has transformed and liberated Cuba.
